Tiivistelmä
Etelä-Savon sosiaali- ja terveyspalvelujen kuntayhtymä Essoten hallinnoima OSSI-hanke, viralliselta nimeltään Etelä-Savon asiakaslähtöinen palveluohjausverkosto ja osaamiskeskus omais- ja perhehoitoon OSSI-hanke 2016–2018, on yksi hallituksen kärkihankkeen ”Kehitetään ikäihmisten kotihoitoa ja vahvistetaan kaikenikäisten omaishoitoa” kokeiluhanke.
Tämä opinnäytetyö liittyi OSSI-hankeen pilottiin, jossa kehitettiin omaishoidon tuen alkuarviointijaksoa. Omaishoitajan ja hänen tarpeidensa huomioimiseksi testattiin pilotissa kahta omaishoitajan elämänlaadun mittaria ASCOT SCT4 omaishoitajille ja CES – Kokemukset omaishoidosta, joista toinen oli tavoitteena ottaa pysyvään käyttöön. Opinäytetyön tavoitteena oli kehittää työväline palveluohjaajille mittareiden käytön tueksi ja työvälineen työstämisprosessilla juurruttaa mittareiden käyttöä palveluohjauksessa. Juurruttaminen toteutettiin osallistamalla palveluohjaajat ja muut pilotissa mukana olevat työvälineen kehittämiseen työpajassa, jossa ideariihi-, Learning cafe- ja Living Lab -menetelmiä soveltaen kerättiin materiaalia, jota myöhemmin pyydettiin täydentämään sähköpostitse.
Työväline muotoutui käsikirjaksi, jossa on pieni johdanto mittareihin, vinkkejä voimavarojen tukemiseksi ja kysymyskohtaisesti apukysymyksiä, huomioita ja keinoja palveluohjaajien käyttöön tuen tarpeiden noustessa mittareiden tuloksista esille.
Käsikirjan käyttökokemukset ja palaute jäivät vähäisiksi pilotissa tapahtuneiden muutosten vuoksi. Käsikirja on muokattavissa helposti vain yhden mittarin kattavaksi, jolloin sitä voidaan käyttää mittarin käytön perehdyttämisessä tai työvälineenä, kun palveluohjaajat haluavat vaihtaa kokemuksia tai kehittää työtapojaan.
Omaishoitajan elämänlaadun mittarin laaja käyttö varmistaa omaishoitajan tarpeiden huomioimisen ja oikeanlaisen tuen tarjoamisen omaishoitotilanteessa. Jatkossa olisi hyvä tarkastella, näkyykö mittarin käyttö hoito- ja palvelusuunnitelmissa. Käyttämällä mittareita systemaattisesti niin arviointi- kuin seurantakäynneilläkin voidaan selvittää järjestetyn tuen vaikuttavuus eli parantaako se elämänlaatua. Elämänlaadun mittarin käyttö voi myös tuoda esiin uudenlaisia tuen tarpeita. OSSI project, officially called Improved family care for older persons and enhanced support for all aged informal carers - OSSI project at South Savo (2016-2018), administered by the South Savo Social and Health Care Authority Essote, is an experimental project of the government key project “Improved home care for older people and enhanced informal care in all age groups.”
This thesis was part of a pilot of OSSI project, in which the assessment period of informal care was developed. In the pilot, two quality of life indicators designed for informal carers, ASCOT SCT4 Carer and The Carer Experience Scale CES, were tested and one of them was intended to be selected for permanent use.
The aim of the thesis was to develop a tool for case managers of service counselling to support the use of the indicators and promote the introduction of the method of using the indicators in the service counselling. For the sake of the promotion, the service managers and other participants of the pilot were involved in the development of the tool in a workshop, where material for the tool was collected using applied methods of brainstorming and Learning café and Living Lab, and later asked to supplement that material via e-mail.
The tool took a form of handbook with a small introduction to the indicators, tips to support the reserves of strength of the informal carer, and, for each question, additional questions, observations and means which the case managers can use if the indicators show any need for support.
The practical experience and feedback of the handbook were insignificant because of changes in the pilot. The handbook is easily adjustable to include only one indicator, which makes it then usable for initiation into the usage of the indicator or as a tool when case managers want to change experiences or develop their working methods.
In the future, it would be reasonable to examine whether the use of the indicator is visible in the support plans. The systematic use of the indicator on both the assessment visits and the follow-up visits to the carer enables finding out the effectiveness of the arranged support.
