Tiivistelmä
Opinnäytetyössä pyritään kartoittamaan hoitajien näkemyksiä ja kokemuksia vallasta psykiatristen sairaaloiden suljetulla osastolla. Tavoitteena on avata keskustelua vallan merkityksestä käytännön hoitotyössä sekä kartoittaa tarvetta käsitellä teemaa sairaanhoitajien koulutuksessa. Opinnäytetyötä varten haastateltiin neljää hoitajaa eri osastoilta. Haastattelut litteroitiin ja tuloksia analysoitiin teemoittelun avulla.
Aineistosta nousi analyysivaiheessa esiin tarve kehittää käsite kuvaamaan valtaa mielenterveys hoitotyössä. Hoidollisen vallan käsite sopi tarkoitukseen hoitajien kertomusten perusteella. Aineiston perusteella hoidollinen valta jakaantuu ammatillisuuteen ja eettisyyteen; joskaan nämä eivät ole toisensa poissulkevia kategorioita. Ammatillisuus käsittää aineiston perusteella alaluokat roolit, hoidollisuuden varmistaminen sekä vastuu ja valta. Eettisyys puolestaan käsittää alaluokat valtaa minimiperiaatteella, avoimuus ja läpinäkyvyys hoitamisen ihanteina, yksilöllisyys ja yhteisöllisyys hoitamisen ihanteina, yksilön etiikka sekä työryhmä ja lait eettisinä työvälineinä.
Sekä aineisto että teoria antavat viitteitä siitä, että valta on hoitotyöhön sovellettuna hyvin moninainen ilmiö. Haastateltavat kokivat, että valtaa olisi tärkeää käsitellä sairaanhoitajien koulutuksessa. Kävi ilmi, että vaikka hoitotyössä käytetään aktiivisesti voimavaraistumisen ja valtaistumisen käsitteitä, itse valta käsitteenä vaatisi perustavanlaatuisempaa tieteellistä soveltamista mielenterveys hoitotyön kenttään. Hoidollisen vallan käsite voisi olla eräs väline vallan tarkasteluun ja tutkimiseen mielenterveyshoitotyössä. This bachelor’s thesis aims at survey the views and experiences of psychiatric nurses about power in the closed wards of psychiatric hospitals nurses work in. My focus is to activate the discussion on the significance of power in nursing practices and investigate if there is a need to converse on the theme in the psychiatric training. For this study four nurses from different wards were interviewed. The interviews were transcribed and analyzed with the method of thematic analysis.
It emerged that there is a need to develop a concept of nursing power to describe the different features of power in the field of nursing. On the grounds of data the nursing power can be divided into the two categories: professionalism and ethics. These categories cannot be totally separated. Based on the data the professionalism has three subclasses: roles, verifying the treatment and at last, power and responsibility. Ethics has five subclasses: ideal of the minimum use of power, ideal of transparency, ideals of individualism and community nursing, the ethics of individual and finally, co-workers and law as the ethical tools.
Both data and theory suggest that power is a diverse phenomenon. The interviewees stated that it is important to deal with the questions of power in the training of psychiatric nurses. It occurred that in nursing the concept of empowerment is widely used but at the same time the concept
power itself would need more basic study. The concept of nursing power could be seen as a tool for investigate and examine power in mental health nursing.
