Syfte med arbete är att kartlägga vad som är viktigt att ha med i en rehabiliterande omvårdnadsplan för att patienten skall få den bästa vården utifrån deras behov. Målet är att genom handledning till sjuksköterskorna på avdelningen framföra vad som är viktigt att tänka på gällande individuell systematisk rehabiliterande omvårdnadsplan. Arbete är ett utvecklingsarbete för Geriatriska kliniken, avdelning Mellangårds. Datainsamlingen utfördes med hjälp av sökning i Ebsco. Examensarbete är en litteratursstudie där den teoretiska delen analyserats utifrån vetenskapliga forskningar och litteraturböcker. Resultatet visar att sjuksköterska har ett stort ansvar över att den individuella omvårdnadsplan upprättas, genomförs, utvärderas och bearbetas. Omvårdnadsplan är som ett hjälpmedel där målet är att säkerställa att de uppgjorda målen eftersträvas och utvärderas och ständigt omprövas. Patienten bör alltid vara delaktig i omvårdnadsplanen. En rehabiliterande omvårdnadsplan skall innehålla konkreta mål för rehabiliteringsprocessen, som patienten skall kunna förverkliga. Mål uppställs individuellt tillsammans med patienten för förbättring eller bevarande av patientens hälsa, funktionsförmåga, motivation och självständighet som skall eftersträvas. Det är inte enbart de fysiska handlingarna som skall dokumenteras utan även aspekter som att stödja och främja patientens självbestämmande och känslan av säkerhet. Målet med rehabilitering bygger på personlig utveckling som skall bidra till ökade möjligheter och ett ökat välbefinnande som innebär att patienten förbättrar sin funktionsförmåga. The purpose is to identify what is important to include into the rehabilitative nursing care plan for patient in order to receive the best care according to his needs. The aim is to express through the nurses' manual in the ward those things, which are important to consider in the real existing individual systematic rehabilitative care plan. This work focuses on the development of the Geriatric Clinic, Mellangårds Department. Data acquisition is performed with the help of EBSCO Research. The submitted thesis is a review of scientific literature; its theoretical part includes an analysis, made by support of scientific research and literature. The results show that the nurses have major responsibility for the individual care plan in its preparing, implementing, evaluating and processing. Health Care Plan is a tool, which aims to ensure the support of the objectives achievement and evaluation and its continuous re-examination together with the patient - the one, who should always be involved. A rehabilitative care plan must include concrete goals for the rehabilitation process, which patients should be able to realize. The goal is individually planned together with the patient depending on improvement or maintenance of patient health, one's functional ability, motivation and independence, which should be sought. Not only physical aspects must be documented, but any aspects of support and promotion of patients' self-determination and sense of security as well. The goal of rehabilitation is based on the personal development, which will contribute to increased possibilities and greater well-being, including improving of functional ability of patient.
