Hallux valgus är en av de vanligaste felställningarna i foten och drabbar var tredje person i västvärlden. Hallux valgus är till stor del ärftligt och är vanligare hos kvinnor än hos män. Etiologin för hallux valgus är mångdimensionell och bara delvis känd. Ett flertal faktorer så som genetiska orsaker, skoanvändning och funktionella fel i foten kan leda till uppkomsten av hallux valgus.
Operativ vård av hallux valgus har blivit allt vanligare och säkrare, men ännu förekommer komplikationer i samband med operationen som kan leda till överkorrigering, förnyad hallux valgus eller uppkomsten av andra problem så som hallux rigidus (styv stortå). Åsikterna om riskerna och nyttan med operativ behandling är delad. I detta arbete har forskningar gällande konservativ behandling undersökts. Dessutom presenteras och sammanställs de nyaste forskningarna gällande riskfaktorer för uppkomsten av hallux valgus. Syftet är att sammanställa den nuvarande kunskapen om etiologin av hallux valgus samt att bilda en klarare uppfattning om ifall tillståndet går att behandla effektivt med konservativ behandling. Arbetet är en systematisk litteraturstudie gjord som ett beställningsarbete från Askelklinikka i Helsingfors.
Ett långt metatarsalben med en rund ledyta visade sig vara den viktigaste genetiska riskfaktorn för uppkomsten av hallux valgus. En lång stortå måste ta emot en större belastning under gången samt hamnar i fel ställning vid användning av framtill smala eller spända skor. Användning av skor av opassande storlek och modell visade sig vara den största icke-genetiska funktionella riskfaktorn för uppkomsten av hallux valgus.
Av de konservativa behandlingsmetoderna som framkommit i de inkluderade studierna visade sig manuell ledmobilisering med traktion vara effektiv för minskning av smärta orsakad av hallux valgus. Användning av ortos hade en kortvarig smärtlindrande effekt som inte bibehölls efter avslutad behandling. Hallux valgus is a common disorder of the foot which affects one third of the population in the Western countries. Hallux valgus is more common among women and is strongly inherited. The etiology of hallux valgus is multifactorial and only partly understood. The use of shoes of wrong size or shape, functional problems of the foot and genetic factors can lead to hallux valgus to develop.
Surgical corrections of hallux valgus have gained popularity and become more satisfying, but overcorrection, recurrence and developing of other disabilities such as hallux rigidus (stiff big toe) do still occur as complications after undergone surgery. The aim of this article was to get a better understanding of the efficacy of conservative treatment of hallux valgus and to present the latest studies about risk factors for developing hallux valgus. This work summarizes the etiology of hallux valgus and questions whether the disorder can be treated efficiently with conservative methods. This is a systematic literature review commissioned by the company Askelklinikka in Helsinki.
A long first metatarsal bone with a curved joint articular surface was one of the most genetically important etiological factors for developing hallux valgus. A long big toe has to take more pressure while walking and bends towards the other toes in shoes with a narrow toe box. Wearing shoes of too small size or with narrow toe box was the biggest non-genetic risk factor in the included studies.
Manual mobilization with traction of the first metatarsal showed to be efficient in reducing pain in hallux valgus. Using orthoses did reduce pain but the effects did not last after the test period ended.
