Näringens betydelse vid läkning av kroniska sår : En litteraturstudie med vårdvetenskapligt perspektiv

Antalet personer som lider av svårläkta kroniska sår kommer att öka. Detta beror på att befolkningens ålder stiger, människorna lever längre med sina bakomliggande sjukdomar och kroppens näringsbehov är otillräckligt och beaktas inte i kosten. Kroniska sår är ett märkbart problem även inom öppna vården och de orsakar en hel del lidande åt patienten. Intresset för examensarbetet har sin grund i ett praktikavsnitt på en sårvårdsmottagning inom öppna vården. Litteraturstudien var ett beställningsarbete och uppdragsgivaren var Lundagatans hälsostation. Den vetenskapliga ansatsen var kvalitativ och analysmetoden innehållsanalys. Litteraturstudien har knutits till ett vårdvetenskapligt perspektiv och som teoretisk referensram har Katie Erikssons vårdvetenskapliga lidande begrepp använts. Syftet med litteraturstudien var att kartlägga forskning kring näringens betydelse vid läkning av kroniska sår och känslor som personer med kroniska sår upplever. Utifrån resultatet har ett lättläst infoblad, med kostrådgivning, riktat till den äldre befolkningen, gjorts till hälsostationens användning. Själva examensarbetet är skrivet så att det går mera in på de olika näringsämnenas betydelse vid läkning av kroniska sår och det lidande personer med kroniska sår upplever, riktat till vårdarna på hälsostationen. Arbetet fokuserar på patienter inom öppna vården. Trots en del brister inom evidensbaserad forskning, kring påvisade effekter i sårläkningen hos personer med kroniska sår, har näringen påvisats spela en central roll för optimal sårläkning. Det har konstaterats att behovet av näringsämnen förändras hos personer med kroniska sår. Att ha ett sår ökar på metabolismen i kroppen och ställer krav på ett adekvat näringsintag för att möta det ökade näringsbehovet och undvika undernäring. Det är inte enbart mängden mat som är viktig, utan kvalitén på den mat som intas. Vårdarens ansvar ligger i att upptäcka personer med kroniska sår som lider av ett dåligt näringstillstånd och skapa en vårdrelation som lindrar lidande på bästa möjliga sätt. Abstract:
The amount of people who suffer from chronic wounds will increase. This is a result of the aging of the population, people live longer with their underlying diseases and the nutritional needs of the body is often not met and not taken into account in the diet. Chronic wounds are also a noticeable problem in non-institutional care and cause a lot of pain and suffering for the patient. Inspiration for the thesis came from an internship at a wound treating reception. The literature study was a commissioned work and the client was Lundagatans hälsostation. The scientific approach used was qualitative study, and the method of analysis was content analysis. The literature study has been combined with the perspective of scientific care and is based on Katie Eriksson’s concepts about suffering. The purpose of the literature study was to identify research in the role of nutrition in healing of chronic wounds, and the feelings that people with chronic wounds experience. An easy to read informational leaflet, aimed at the elderly population, was produced based on the results, for the health centre to distribute. The thesis itself is more about the purpose of the specific nutrients and the suffering of people with chronic wounds, and is thus aimed for the nurses at the healthcare center. The work focuses on patients in non-institutional care. Despite some shortcomings in the evidence-based research on the effects, nutrition has been proved to play a central role in the optimal healing of wounds. It has been found that the need for nutrients is altered in people with chronic wounds. Having a wound increases metabolism and adequate nutrition is required to meet the increased nutritional needs and avoid malnutrition. It is not only the amount of food that is important, but also the quality of the food itself. The nurse’s responsibility is to identify people with chronic wounds that suffer from malnutrition and alleviate suffering in the best possible way.
