Abstract
The examination of bone marrow is a standard procedure in the diagnosis of haematological disorders, disease monitoring and assessment of treatment response. However, the examination of bone marrow samples is labourintensive and the number of specialists in the field of clinical chemistry responsible for ana-lysing patient samples is predicted to decrease in the future. Computer-aided diagnosis is recognised as one solution to support reducing resources and speed up the process. Despite the fast development of the deep learning methods and progress in medical domain, there does not yet exist a framework that would al-low a large-scale automatic analysis of bone marrow samples. In this thesis, we present a novel machine learning-based framework for auto-mating the analysis of MGG-stained bone marrow aspirate samples from whole slide images, scanned with a 10x-magnifying objective. We build a six-phase framework for recognising the sample region and the centre of the sample from the whole slide image, estimating the area of stromal fragments, red blood cells and eukaryotes, the number and area of megakaryocytes and lipid droplets, and the sample cellularity. Finally, a report template was constructed for the use of physicians. In addition, a comparative test for two preparation types, squash, and wedge, was performed on a small sample set. In the first five phases, we generated an image dataset and trained a convolutional neural network. U-net was applied to the segmentation tasks, including the sample segmentation and the segmentation of stromal fragments, red blood cells and eukaryotes. Mask-RCNN was applied to the instance segmentation tasks, including the recognition of centre region, lipid droplets and megakaryocytes. The sample cellularity was estimated by extracting various cellular features from 200 samples and by training a logistic regression model. All the models showed sufficient performance and demonstrated to be applicable in large-scale examination of bone marrow samples. In general, the performance of the convolutional neural network models was limited by the ambiguity of the target classes in the annotation phase, demonstrating the importance of reliable labelling.

Luuytimen morfologinen arviointi on rutiinitutkimus hematologisten häiriöiden diagnosoinnissa, taudin seurannassa ja hoitovasteen arvioinnissa. Luuydin-näytteiden tutkiminen on kuitenkin työlästä ja potilasnäytteiden analysoinnista vastaavien kliinisen kemian asiantuntijoiden määrän ennustetaan vähenevän tulevaisuudessa. Tietokoneavusteinen analytiikka on yksi ratkaisu helpottamaan resurssien vähentämistä. Huolimatta syväoppimismenetelmien nopeasta kehityksestä ja edistyksistä lääketieteellisissä käyttötapauksissa, luuydinnäytteiden laajamittainen automaattinen analysointi ei ole vielä mahdollista. Tässä opinnäytetyössä esittelemme uuden, koneoppimiseen perustuvan viitekehyksen MGG-värjättyjen luuytimen aspiraationäytteiden automaattiseen tutkimiseen 10-kertaisella suurennuksella skannatuista kokopreparaattikuvista. Rakennamme kuusivaiheisen viitekehyksen näytealueen ja näytteen keskusalueen tunnistamiseksi kokopreparaattikuvista, stroomafragmenttien, punasolujen ja eukaryoottien pinta-alan, megakaryosyyttien ja lipidipisaroiden lukumäärän sekä näytteen solukkuuden arvioimiseksi. Lopuksi laadimme raporttipohjan lääkäreiden käyttöön. Lisäksi suoritamme vertailevan testin kahdelle eri valmistetyypille, puriste- ja sivelyvalmisteelle. Viidessä ensimmäisessä vaiheessa loimme kuvatietojoukon konvoluutioverkkojen kouluttamista varten. U-net arkkitehtuuria sovellettiin segmentointiteht-viin, sisältäen näytteen sekä stroomafragmenttien, punasolujen ja eukaryoottien segmentoinnin. Mask-RCNN-arkkitehtuuria sovellettiin instanssisegmentointi-tehtäviin, mukaan lukien näytteen keskusalueen, lipidipisaroiden sekä megakaryosyyttien tunnistamiseksi. Näytteen solukkuuden arviointi toteutettiin määrittämällä sytologiset piirteet 200 näytteestä, ja kouluttamalla logistinen regressiomalli. Kaikki mallit osoittivat riittävää suorituskykyä ja osoittautuivat soveltuviksi laajamittaiseen luuydinnäytteiden tutkimukseen. Konvoluutioverkkojen suorituskykyä rajoitti kohdeluokkien epäselvyys annotointivaiheessa, mikä korostaa laadukkaan annotointityön tärkeyden konenäkömallien kehityksessä.
