Using Smartphone Barometer Sensor Data to Measure Performance Outcomes in Clinical Trials

Abstract
Enabling remote assessments of electronic performance outcomes (ePerfOs) in clinical trials could provide more descriptive measures and allow an increased number of tests per trial. Novel instruments using smartphone software and onboard inertial sensors have been proven feasible for remote collection of ePerfOs. However, these instruments are limited in their applicability, and there remains a need for instruments that capture movement in activity tests that involve a height change (e.g., stair-climb tests). This thesis aimed to evaluate the suitability of the smartphone barometer for height change capture in at-home ePerfO tests using a smartphone. Sensor stability was assessed by atmospheric pressure (AP) measurements at constant height in 3 conditions: indoor with windows closed (INWC), indoor with windows open (INWO) and outdoors (OUT) (60 s, 3 repetitions). The ability to detect height transitions was assessed by holding the smartphone at a constant height for 20 s, moving the smartphone to a second height of 0.6 m, and retaining the second height for the rest of the measurement (60 s, 8 repetitions). Height transition periods were estimated by a self-developed algorithm, and the height gained was estimated by averaging AP data before and after the transition period. Mean [SD] for estimated transition start was 20.43 [0.39] s, 20.58 [1.81] s, and 20.41 [0.62] s, for INWC, INWO and OUT respectively. Mean [SD] for estimated height gained was 0.65 [0.13] m, 0.66 [0.15] m and 0.81 [0.19] m. The study results indicate that the smartphone barometer is able to detect a transition period and estimate the height gained during a transition of 0.6 m in indoor conditions. As a 0.6 m height transition corresponds to a climb of 4 steps within a standard staircase, study results suggest that the method can be useful for development of remote PerfO tests using a smartphone.

Att möjliggöra bedömning av prov i fysisk förmåga (eng. Performance Outcomes") på distans under kliniska undersökningar kan driva till mer beskrivande mått av patientens fysiska tillstånd, samt tillåta ett högre antal utförda bedömningar under en undersökning. Nya smarttelefonsapplikationer har möjliggjort genomförandet av ett antal av dessa prov från patientens hem genom användning av smarttelefonens inbyggda positionssensorer till att mäta patientens rörelse under provet. Dessa mätinstrument är dock begränsade till mätning av horisontell rörelse, och det saknas lösningar för att mäta patientens rörelse i test av fysisk förmåga som inkluderar vertikal rörelse, till exempel klättrandet av trappor. Denna studie har undersökt användbarheten av smarttelefonens lufttryckssensor för mätning av höjdskillnader som ingår i test av fysisk förmåga. Sensorns stabilitet undersöktes genom lufttrycksmätningar i tre olika förhållanden: inomhus med fönster stängda (IFS), inomhus med fönster öppna (IFÖ) och utomhus (UTE) (60 s, 3 repetitioner). Sensorns förmåga att registrera och mäta en höjdskillnad undersöktes genom att hålla smarttelefonen vid en konstant höjd i 20 s, flytta den 0.6 m, varefter telefonen hölls stilla vid den nya höjden (60 s, 8 repetitioner). Med användning av en självutvecklad algoritm estimerades tidsperioden då telefonen var i vertikal rörelse, och höjdskillnaden estimerades genom att beräkna genomsnittliga lufttrycket under tidsperioden innan och efter rörelse. Medelvärdet [SD] för estimerad start av rörelse var 20.43 [0.39] s, 20.58 [1.81] s, och 20.41 [0.62] s för IFS, IFÖ och UTE mätningar. Medelvärdet [SD] för uppmätta höjdskillnader var 0.65 [0.13] m, 0.66 [0.15] m och 0.81 [0.19] m. Resultaten indikerar att smarttelefonens lufttryckssensor är användbar för att registrera och mäta en rörelse i 0.6 m höjdskillnad inomhus. Eftersom en 0.6 m höjdskillnad är jämförbar med fyra trappsteg i en genomsnittlig trappuppgång, antyder resultaten att metoden kan användas i utvecklandet av applikationer som tillåter bedömning av prov i fysisk förmåga på distans.
