Tiivistelmä
Lukuisat munuaissairaudet voivat johtaa munuaisten krooniseen vajaatoimintaan, jossa potilaan oma munuaistoiminta ei enää riitä ylläpitämään elämää. Tällöin hänen tulee turvautua munuaisensiirtoon tai erilaisiin läpi elämän jatkuviin dialyysihoitoihin. Munuaisdialyysi, jossa elimistön aineenvaihdunnan seurauksena vereen kertyneet kuona-aineet ja ylimääräinen neste siirtyvät membraanin läpi dialyysinesteeseen, on kuitenkin kallis ja monta kertaa viikossa suoritettava hoitotoimenpide.
Työssä tarjotaan lukijalle uusimpaan tutkimustietoon perustuva ymmärrys membraanien roolista munuaisdialyysistä. Yleisimmät hoitomuodot käydään ensin läpi yksityiskohtaisesti, jotta munuaisdialyysistä saadaan selkeä kokonaiskuva, minkä jälkeen tarkastellaan itse membraaneja ja niiden soveltamista. Membraanien historian ja kehityksen tarkastelun kautta käy ilmi, miten dialyysimembraanit ovat vuosien saatossa kehittyneet nykyhetkeen. Työn tarkoituksena on lisäksi antaa tarkka kuva nykyisin markkinoilla olevien dialyysimembraanien fysikaalisesta ja kemiallisesta rakenteesta, niiden toimintaperiaatteesta, valmistusprosessista sekä minkälaisia ongelmia ja haasteita tutkijoiden tulee selvittää niiden osalta tulevaisuudessa.
Koska uusille lääketieteellisille innovaatioille on tulevaisuudessa tarvetta, syvennytään tässä katsauksessa lisäksi erityisesti tulevaisuuden dialyysimembraaneihin ja hoitomenetelmiin sekä potentiaalisiin tutkimuskohteisiin. Tutkielma pyrittiin tekemään suomalaisen terveydenhuollon näkökulmasta, jotta sen mahdolliset hyödyt ovat mahdollisimmin helposti sovellettavissa. Numerous kidney diseases can lead to chronic kidney failure, where the patients’ own renal function is no longer sufficient to maintain life. Patients must then resort to renal transplantation or to various dialysis treatments which will last for the rest of their lives. Renal dialysis, where waste products and extra fluid accumulate in the blood due to metabolism and diffuse through the membrane to dialysis fluid, is a costly treatment that needs to be administered several times a week.
This work provides the reader with the latest research knowledge of the membranes’ role in kidney dialysis. The most common forms of treatment are first reviewed in detail to provide a clear overall picture of renal dialysis, after which the membranes themselves and their application are examined. Through the review of membrane history and development, it emerges how dialysis membranes have evolved over the years into the present. The aim of this thesis is to give an accurate picture of the physical and chemical structure of the dialysis membranes currently on the market, their operating principle, their manufacturing process and what kind of problems and challenges the researchers will need to solve in the future.
Since new medical innovations are needed in the future, this review will further focus on the future of dialysis membranes and therapeutic methods as well as on the potential research topics. This thesis was aimed to be written from the perspective of Finnish health care system so that its potential benefits are most easily applied in Finland.
